Young Adult Survey

The ADAPT (Alcohol & Drug Abuse Prevention Team) Coalition of Columbiana County seeks to plan and implement strategies to prevent and reduce youth substance use.

You are being asked to participate in a Young Adult Survey which is being conducted by The ADAPT Coalition and the Underage Alcohol Prevention Initiative of the Family Recovery Center to determine the use and perceptions of young adults about alcohol use.  This survey is anonymous. No one, including any staff or volunteer member of the ADAPT Coalition will be able to associate your responses with your identity. Your participation is voluntary. You may choose not to take the survey, to stop responding at any time, or to skip any questions that you do not want to answer. You must be at least 18 years of age to participate in this study. Your completion of the survey serves as your voluntary agreement to participate in this needs assessment project and your certification that you are 18 or older. If you have questions about the survey or would like information about the ADAPT Coalition, please contact Brenda Foor, Coalition Coordinator at 330-424-0531, or e-mail at or Luke Newton at 330-+424-0531 or email him at .

In this survey, the phrase "Underage Youth" refers to persons under the age of 21. A drink is defined as a glass of wine, a bottle or can of beer, a shot of liquor, or a mixed drink. References to the use of alcohol do not include a few sips for religious purposes.

Please visit to take the survey.
